Complete Diabetic Systems
is totally committed to our main goal of helping Medicare patients
and those with private insurance avoid the out-of-pocket expense
they incur when purchasing diabetic supplies on their own. Approved
by Medicare and registered with the State of Florida, we work with
Medicare and most private insurance companies (other than HMO and
certain PPO plans) to alleviate the cost of these vital items. In
order to make it as easy for you as possible, we ship your supplies
directly to your home and then bill Medicare or your insurance company.
If
you are a Medicare recipient, you must have Part B coverage. You
are responsible for your annual $100 deductible. You may choose
to meet your deductible through your doctor or through us. Also,
you may pay us either on an installment basis, or in full by personal
check or money order. Finally, you will also be responsible for
any co-payment required by your insurance plan.
If
you have private insurance, you must also have your deductible satisfied
before we can help you. We offer the same opportunity to satisfy
your deductible as patients on Medicare. However, we regret that
we cannot work with HMO plans at this time. Certain PPO plans will
allow us to service patients, although sometimes at a reduced rate
of coverage.
Under
Medicare Part B coverage, we can provide glucose monitors (Medicare
will reimburse for a new meter every five years,
test strips, and lancets. Private coverage usually takes care of
glucose monitors, test strips, lancets, alcohol swabs, insulin,
and syringes. Other diabetic supplies not mentioned are available
if obtainable through our suppliers, and if you have the necessary
coverage.
In
order to gain the benefits of our services, you must complete the
customer enrollment information form. This is an electronic form
that will automatically and confidentially forward your information
to us, so that we can start you on our program as quickly as possible.
